Special Instructions:
The Connecticut Department of Energy and Environmental
Protection (DEEP) is tasked by state statute (22a-602) to
manage EPCRA data for the Connecticut State Emergency
Response Commission (SERC). DEEP prefers that each
facility use EPA's Tier2 Submit software application for
the current reporting year. DEEP will accept Tier2 Submit
electronic file submissions via email attachment to dep.ctepcra@ct.gov,
or submissions on cd with one zipfile submission to SERC
address with a company cover letter and label on the CD,
or paper through regular mail. DEEP will reply in kind to
submissions (of Tier2 Submit electronic files only) made
by email with an acknowledgment of receipt only, if one is
requested. Email acknowledgment does not constitute
validity of the data and a follow-up notification will be
made to the facility if DEEP encounters problems with

The CERC requires electronic submission of Tier2 Submit forms and payments. Payments may be made online using electronic check or credit card, or payment may be invoiced and mailed in separately. The online submission will require the use of the EPA Tier2 Submit program for Tier2 preparation. A new version of Tier2 Submit is created around November of each year. Download the latest version of Tier2 Submit.
Most LEPCs and Fire Departments require a hard copy report. However, Fayette County (Lexington) and Jefferson County (Louisville) LEPCs require electronic submission of Tier2 Submit files. Send Fayette County submissions by e-mail to sjackson@lexingtonky.gov and Jefferson County submissions by e-mail to jim.bottom@louisvilleky.gov.
EPCRA allows the exact location of hazardous substances at a facility to be kept confidential. Facilities wishing to keep this information confidential must submit a hard copy of the Tier2 Confidential Location Form. Companies operating multiple facilities are requested to submit a hard copy of the Multiple Facility Fee Form in addition to the Tier2 form and fees.

As of 1 March 2014 (reporting year 2013) The State Emergency Response Commission will only accept electronic submissions through E-plan; after which, no paper reports will be accepted.

you don’t need a user ID and Password to start. If
you have trouble with the website you may notice a link on
the left side of the page for ‘online training’ or use the

contact the webmaster.
The E-plan website has common Safety Data Sheets in a
digital library; so, attaching additional SDSs is
unnecessary for many common chemicals.
Reporting facilities may also attach contingency plans,
maps, photos, etc. in the “additional information”
section.
Currently only Rutherford and Madison Counties require
reporting through E-plan.

Texas is requiring facilities to use Tier2 Submit and
requests that electronic Tier Two data files (created out
of the Tier2 Submit version, not scanned pdf or word
documents) be submitted via email to T2Sdatafiles@dshs.state.tx.us.
Please include a contact name and telephone number in the
body of the email for reference. If emailing is not
an option, then electronic data files may be submitted on
CD or floppy disk via U.S. mail. To avoid confusion,
please do not send duplicate submissions of your Tier Two
Report in any format with your filing fee. The TXT2
Number must appear on the Tier2 Submit State Fields
screen, the check or money order for the fee payment, and
in the email subject line. Fee payments must be sent
by U.S. Mail to the address provided on the Filing Fee

Note: Submission of your electronic data file to
DSHS Tier II Chemical Reporting Program serves as
notification to the State Emergency Response Commission
(SERC) for Tier II data. Facilities need not submit

Special Instructions:
The Utah Department of Environmental Quality (UDEQ) is
tasked by state statute to manage EPCRA data for the Utah
State Emergency Response Commission (SERC). UDEQ prefers
that each facility use EPA's Tier2 Submit software
application for the current reporting year. UDEQ will
accept Tier2 Submit electronic file submissions via email
attachment, or submissions on diskette, or paper through
regular mail (UDEQ does not offer on-line direct internet
form submission). UDEQ will reply in kind to submissions
(of Tier2 Submit electronic files only) made by email with
an acknowledgment of receipt only. Email acknowledgment
does not constitute validity of the data and a followup
notification will be made to the facility if UDEQ
encounters problems with data. When using the Tier2 Submit
application, UDEQ requests that each facility enter its
Utah Tier II State ID number (A facility submitting Tier
II for the first time should contact UDEQ to be assigned a

The Wyoming State Emergency Response Commission (SERC)
will not accept the American Petroleum Institute (API)

when reporting chemicals. Please be specific on chemical
amounts in pounds and gallons when reporting. The Wyoming
SERC encourages facilities to report the facility location
in longitude and latitude to assist local first

In addition to the electronic submission file from Tier2
Submit, the Wyoming SERC requests a completed electronic
cover sheet and ONE page of the paper report bearing the
official contact information and original signature. The

needed by the local fire department and/or LEPC. The
