Complete the following form to register for an EPA ORACLE ID, which will allow you to connect directly to the Envirofacts ORACLE database using SQL over a secure Internet connection. This form must be filled out completely before your request for access can be processed.

If you are a State Agency user (employee, contractor, or grantee), or if you are an EPA contractor or grantee, you will also need to contact a US EPA manager (branch chief or higher) that is familiar with your data needs and is willing to sponsor your request for direct access. If you are a Federal contractor or grantee, you will also need to supply the name of a Federal manager (branch chief equivalent or higher) for sponsorship. The sponsor's name, office, title, and contact information must be included on this registration form or your request will not be processed.
