RAD Reporting Form Downloading Instructions

If you have problems opening the Excel version of the reporting form, please follow these instructions:

1) Open Excel
2) Select "Tools" (make sure you see the entire menu)
3) From the drop-down menu, select "Macros"
4) From the sub-menu, select "Security"
5) Under the "Security Level" tab, de-select "High" and select "Low"
6) Press "OK"
7) Open the RAD Reporting Form. After opening the file and saving it to your computer, it is recommended to set the security level back to “High” following the same steps above.
