- For each controlled substance, reporting forms and guidance can be downloaded in either Word or Adobe Acrobat files (About PDF). All reporting forms should be signed and submitted to EPA either by mail or fax. The two certification forms do not need to be submitted to EPA.

For Methyl Bromide Laboratory Suppliers: Please use the Essential Use Holder and Laboratory Supplier Quarterly Report located with the Class I reporting forms to report on the amount of methyl bromide purchased from producers and/or importers and sold to laboratory customers. Please note that you will need to manually enter methyl bromide as a chemical in column A of Section 4 and Section 5 of the reporting form.
