2009 National Leadership Award Winners - Region 5 Partners
2009 U.S. EPA Region 5 Partners for Asthma Action Program
U.S. EPA Region 5 is fortunate to have many outstanding asthma initiatives. In order to honor this excellent work, Region 5 initiated its Partners for Asthma Action Recognition Program for the first time in 2007 and recognized additional partners in 2008 and 2009. This recognition is for programs that are less than five years old, but have shown great initiative and innovative approaches, and are already achieving positive results. Each of the programs listed below exhibits at least 4 of the Program Elements. Each recognized partner received a scholarship to attend the Third National Forum in Washington DC as well as a certificate of recognition.
Fox Valley Asthma Coalition – Fox Valley, WI
The Fox Valley Asthma Coalition is a non profit group striving to reach individuals who may or may not have access to education for asthma. The coalition promotes asthma education in schools, at sporting events and in the local community and has been meeting monthly since January of 2006. The Fox Valley Asthma Coalition chose three priority areas: Clinical Care, Education and Environment. Goals for the current grant year are to provide the ALA programs for school, coaching, daycare staff and children. The Fox Valley Asthma Coalition is fortunate to have representation from area health care systems, clinics, health departments, and schools. Members in the Coalition include physicians, nurses, pharmacists, respiratory therapists, medical assistants and pharmaceutical representatives. The focus is to provide asthma education to target populations, coaching staff of youth sporting groups, daycare and school staff, and children. YMCA of the Fox Valley sponsors a Healthy Kids Day at five locations every May with various health-related activities and information booths. One current project is to participate at three sites providing education and spirometry screening.

Delaware County Asthma Coalition – Delaware County, IN
The Delaware County Health Department (DCHD) is funded through an asthma grant from EPA and Indiana State Health Department to provide asthma education in homes and schools. In February 2009, the Asthma Program Coordinator met with physicians, “Family Residence”, Head Start case managers, pediatric and teen case managers, Open Door Clinic and Tobacco-Free Coalition to discuss a partnership with DCHD, which focused on referring patients in need of asthma intervention and Coalition membership. The program exemplifies several of the key drivers by working with the respiratory therapists and emergency room physicians at Ball Memorial Hospital and Open Door Clinic and setting up an important referral system. The short term goals are to increase referrals and continue to follow up and educate new clients. The future goals include growing the asthma coalition and sustaining its efforts.

South Central Asthma Coalition – Glencoe, MN
The Glencoe group, representing the South Central MN Asthma Coalition, has been meeting for two and a half years. They have strong champions in a mother that lost her son due to asthma and a very active local pediatrician. The group has a successful asthma walk in their community that provides funds to support asthma trigger reduction. They also provide education to daycares and schools. Their long term goal is to have a local fund that is sustainable which would pay for nurse home visits to assist low-income families reduce environment triggers.

Illinois Asthma Partnership (IAP) – Central Region
This newly formed, effective partnership includes five local coalitions within the 33-county area of central Illinois. In the past, the coalitions have worked independently but joined forces in October of 2007 to become IAP-Central Region. The regional program is already strong in many of the key drivers. They have strong community ties since each local coalition is deeply rooted in the community and is providing outreach to residents through asthma education, parent support groups, and asthma screening/outreach events. Local medical providers are active in offering asthma education expertise during many speaking engagements within the community to numerous audiences, including parents/caregivers of children/family members with asthma, school coaches, student groups, and other medical care professionals. They work in coordination with American Lung Association of Illinois to promote and staff an asthma camp each year over the Labor Day weekend. Their long term goals include reducing hospitalization rates and emergency room visits by establishing timelines to promote and provide various educational programs for the health care community, school systems, and the community at-large.

Northwest Ohio Asthma Coalition – Northwest Ohio
The Northwest Ohio Asthma Coalition is a group of healthcare providers, public health professionals, educators, government representatives, consumers and others brought together by the common goal of improving the quality of life for people with asthma. They serve an eighteen county area in Northwest Ohio. They have two successful projects that exemplify several key drivers.First, the Healthful Schools project shows strong community ties and tailored environmental interventions by working collaboratively with the Toledo Public School system to monitor and improve air quality in four inner city schools. Second, Strike Out Asthma is an educational outreach event for 200 inner city children affected by asthma and their families. Their goals include: recruit active participation or offer cross membership with pollution control and similar environmental organizations and gather more participation from counties beyond a 50-mile radius of Toledo.

Model Programs
Community-based programs are improving the health and well-being for asthma patients across the country by providing quality asthma care. U.S. EPA created the National Exemplary Awards Program to recognize and highlight successful asthma programs and accelerate the adoption of best practices. The National Exemplary Award recognizes community-based asthma programs that have the following Key Drivers and Essential Program Elements:- Committed program champions

Great Lakes Region Asthma Forum - 2007
In order to complement the National Asthma Forum efforts, Region 5 co-hosted, along with the American Lung Association of Metropolitan Chicago and the American Lung Association of the Upper Midwest, a Great Lakes Region Asthma Forum in February, 2007. Over 140 partners from around the six Region 5 States came together to learn from each other and from Model Programs (the 2007 National Exemplary Award Winners) about successful community-based asthma programs.
