How to Submit a Freedom of Information Act (FOIA) Request to the Region 5 Underground Injection Control Branch
If you wish to receive copies of items from government files, you should submit a Freedom of Information Act request, referred to as a FOIA (pronounced "foya"). For information from the Region 5 UIC Branch, please submit your request either by email to Ms. Mary Redding or by regular mail to

Your request should mention that it is submitted under the Freedom of Information Act (a reference to "FOIA" is sufficient) and should provide as detailed a description of the information in which you are interested as you can. For example, if it is about a specific company, a specific county or a specific injection well or a specific period of time, please include that information.
The charge for this service depends upon the amount of work it takes to fulfill your request. Staff time is billed at the rate of $2 or $5 for senior staff in one-quarter hour increments (that is, $8/hr or $20/hr), depending upon the complexity of your request. Copies are made for $0.15 per page and diskettes are $1.00. (There are additional charges for special services.) If the total comes to less than $25.00, the fee is waived. In addition, for requests from educational or non-commercial scientific institutions, or from the media, the first 100 pages are free.
