Instructions and Form for Hazardous Waste Generators, Transporters and Treatment, Storage and Disposal Facilities to Obtain an EPA Identification Number (EPA Form 8700-12/Site Identification Form)

How to Look Up a Site ID Number

Use our search tool to find a site ID number for a facility.

Federal regulations require large and small quantity generators of hazardous waste to obtain an EPA Identification (EPA ID) number using the Subtitle C Site ID Form (EPA Form 8700-12) and to submit the completed form to the authorized state agency or EPA regional office if the state is not authorized to implement the Resource Conservation and Recovery Act (RCRA) Subtitle C program. The Site Identification Form (EPA Form 8700-12) includes information such as facility’s name and address, contact information and a description of hazardous waste activities conducted at the site.

Small Quantity Generator (SQG) Re-Notification

EPA requires SQGs to re-notify EPA or their state environmental agency as to their generator status every four years. EPA first required SQGs to re-notify by September 1, 2021, and requires them to re-notify again by September 1, 2025. Learn more.

Documents and Instructions

EPA designed the instructions for EPA Form 8700-12 to help businesses determine if they are subject to requirements under RCRA for notifying EPA of their regulated waste activities. Also, these instructions will assist in obtaining an EPA ID number by completing and submitting EPA Form 8700-12, either electronically or on paper, for initial notifications or in revising the form if required to submit a subsequent notification.

The entire packet containing all three forms, the Notification of RCRA Subtitle C Activities (Site Identification Form 8700-12), the Hazardous Waste Report (Biennial Report Form 8700-13A/B) and the Part A Permit Application (8700-23), including the instructions, is available as one PDF (2.26 MB).

EPA is making available an excerpt of the full document (pdf)(1.48 MB) containing only the information pertinent to the RCRA Subtitle C Site Identification Form, summarized below:

  • Introduction
  • Notification of RCRA Subtitle C Activities (Site Identification Form).

Applying for an ID Number Electronically

EPA has electronic reporting options available for the 8700-12/Site ID form in states that have opted in to using the electronic system. Please check with your state environmental agency to find out if the electronic MyRCRAID system is available to you. If available, you can access MyRCRAID through the RCRAInfo system.

For Help with this Form or Where to Send it

Please contact your state or, where appropriate, regional representative. Access contact information on this webpage.

Note: Very small quantity generators of hazardous waste are not required by federal EPA to obtain an EPA ID number or submit a notification form but may be subject to state-specific reporting requirements. All generators should consult with their state agencies to clarify the specific reporting requirements for that state.
