How do applicants reflect the 6th year of funding on the SF424A form, as there is only room for 5 years?

Do we need a Good Laboratory Practice-certified commercial lab(s) for the chemical analyses? We believe the answer will be no.
The NOFO does not include a requirement regarding Good Laboratory Practice certification for laboratories.
