Fumigant Management Plan - Phase 1 Templates

EPA created the templates in PDF and Word formats for the different soil fumigants.  Users can choose which format they prefer.  The PDF templates are for users to print and fill out by hand and the Word templates are for users to fill out on the computer.  The agency also created samples of filled out FMPs for some of the chemicals.  Since label requirements vary based on the soil fumigant, please use the appropriate chemical-specific templates.  

Most pesticide labels will require Phase 2 templates.
