Applicator Certification Form under the EPA Plan

Commercial and private applicators wishing to apply restricted use pesticides (RUPs) under the EPA Plan for the Federal Certification of Applicators of Restricted Use Pesticides within Indian Country (EPA Plan) must complete and submit an application form, included below. Commercial and private applicators with an underlying certification from a state, tribe, or other federal agency must submit a copy of both sides of their certification along with the completed application form. By doing so, applicants are agreeing to the following:

  • All commercial and private applicators must be at least 18 years old.
  • Direct supervision of noncertified applicator requirements will be followed if it is allowed by the underlying certificate.
    • Private applicators who have completed the EPA-administered training must follow federal minimum requirements for direct supervision of noncertified applicators (see Who Needs to Apply for Pesticide Applicator Certification in Indian Country under the EPA Plan for more information about direct supervision).
  • Commercial applicators must follow all recordkeeping requirements in 40 C.F.R. Part 171.303(b)(7)(vi) (see Applicator Recordkeeping Requirements for more information).
  • Applicators attest that they understand they are responsible for following any tribal codes, laws, policies, regulations, etc. relevant to the application of pesticides, including RUPs.

Applicators understand that certain information will be posted to the EPA website, including, but not limited to, name and address. The information posted will be dependent on whether the applicator is a commercial or private applicator (see List of Applicators).

  • A false statement in this application, including regarding the completion of training, may be grounds for denial, suspension, or revocation of certification and may be punishable by fine or imprisonment (U.S. Code, Title 18, Section 1001).
