SUBCHAPTER 7. INSPECTION STICKERS AND MONTHLY TAB INSERTS FOR WINDSHIELD AND TRAILER/MOTORCYCLE (OAC 595:20-7)

595:20-7-1. General
(c) Inspection stations will order a sufficient quantity of monthly and yearly tab inserts, one (1) through twelve (12), which indicate the month and year each vehicle is due for reinspection.
Example:
If a vehicle is inspected in April of 1993 the monthly tab insert numeral 4 and year tab insert numeral 4 will be placed on the face of the sticker. This indicates the vehicle was inspected in the month of April 1993, and is due for re-inspection in April of 1994. Tab inserts will be supplied with each sticker order if requested. In the event a station runs short of the tab inserts, the monthly tab portion of sticker order form can be filled out and mailed to this Department, and an additional supply of the inserts will be forwarded at no charge.
(f) The inspection sticker will be placed inside the windshield approximately five inches (5") up from the lower left hand corner and attached by its own adhesive. In the event other official stickers are in the lower left hand comer, such as stickers issued by military bases, or valid stickers issued by another state, the Oklahoma vehicle inspection sticker will be placed as near the lower left hand comer as possible.
595:20-7-2. Inspection certificate
(a) The inspection certificate form is in duplicate and is to be completely filled out by the inspector. If vehicle passes inspection, the carbon copy is to be given to the owner/operator of the vehicle. The original copy is to be retained in the book as a permanent record for one (1) year.
(1) Fill in owner’s name, address, city and state.
(2) Fill out year, make and model of vehicle.
(3) Fill in ID number or serial number taken from the vehicle.
(4) Fill in license number and state.
(5) Fill in date of inspection and mileage reading.
(6) Fill in station number, inspector’s number, and sign the certificate.
(7) Check only the items that are corrected by the inspection station.
(A) If the vehicle passes without correction, leave items blank.
(B) If the vehicle is repaired, check items repaired.
(8) Repair costs will be shown on each item that is repaired or adjusted by the inspection station.
(9) Total repair costs, inspection fee, and total cost will be shown on the inspection certificate.
(10) Record all tire tread measurements.
(11) Record previous inspection sticker number, and year it was issued.
595:20-7-3. Rejection receipt - Form VID 44
The rejection receipt is a duplicate form. It is to be filled out when one or more items of equipment do not pass inspection, and the owner or operator wishes to repair it himself or have it repaired elsewhere. The duplicate copy will be issued to the driver or owner of the vehicle and original copy will remain at the inspection station. If the vehicle is returned within thirty (30) calendar days, only the rejected items will be reinspected at that time, at no additional fee. However, if it has been longer than thirty (30) calendar days or vehicle is rejected the second time, the entire vehicle must be reinspected and another $5.00 fee charged.
595:20-7-4. Station monthly report - Form VID 21
(a) Each official inspection station shall maintain the monthly report book listing each vehicle inspected. These forms will be furnished by the Department. The information to be recorded on this form is taken from the inspection certificate. This form is to be prepared in duplicate. The original copy must be forwarded to the Vehicle Inspection Division by the 10th day of each month following the month for which the report was completed. The carbon copy of the monthly report will be retained by the inspection station for a period of one (1) year after the last inspection recorded.
595:20-7-5. Signature card - Form VID 17
(a) The signature card will be signed by no more than (3) authorized persons and then forwarded to the Vehicle Inspection Division where it will be kept on file. This signature card is to be used for comparing or verifying signature(s) on requests for inspection stickers.
595:20-7-6. Request for inspection stickers - Form VID 19
(a) The request for inspection stickers form shall be completed in duplicate whenever a request for inspection stickers is being submitted by an inspection station owner or authorized person. One copy should be retained by the inspection station for their records.
595:20-7-7. Request for refund - Form VID 25
(a) The request for refund form is to be filled out only when there is a sticker change. Obsolete and unused stickers must be forwarded to this Department along with the request for refund form before a refund will be made. One copy of the request for refund form should be kept by the inspection station for their records.
