Forms for Registration and Reporting Under the FARR

The following forms are for owners and operators of air pollution sources on Indian reservations in Idaho, Oregon, and Washington that are subject to EPA's Federal Air Rules for Reservations.

  • Report of Relocation Form (pdf) (446.5 KB)
    Owners/operators must submit a report of relocation in writing no later than 30 days prior to relocation of the source.
  • Report of Closure Form (pdf) (204.21 KB)
    Owners/operators must submit a report of closure in writing within 90 days after cessation of all operations (except for regular seasonal closures).
  • Report of Change of Ownership Form (pdf) (337.14 KB)
    Owners/operators must report change of ownership within 90 days after the effective date of change.
