Alleged Violations: Respondent failed to report a range of annular pressure fluctuation for two underground injection wells, operated one underground injection well without maintaining a minimum pressure of 100PSI on the annular space, and failed to report monitoring data over a two-month period.

Proposed Order and Penalty: $92,000
