The public reporting and recordkeeping burden for this collection of information is estimated to average 15 minutes per response. An agency may not conduct a collection of information without a valid OMB control number. The OMB control number for this collection is OMB 2050-0179. Send comments on the Agency’s need for this information, the accuracy of the provided burden estimates, and any suggested methods for minimizing respondent burden, including through the use of automated collection techniques to the Director, Collection Strategies Division, U.S. Environmental Protection Agency (2822T), 1200 Pennsylvania Ave., NW, Washington, D.C. 20460. Purpose for this Questionnaire: This survey is meant to help understand community member concerns about the cleanup and how EPA staff can make it possible for community members to participate in the planning and decision making process.  

Instructions: You may be asked some of the questions on this questionnaire. This questionnaire should take no more than 15 minutes to complete, and responses are voluntary. 

The U.S. Environmental Protection Agency (U.S. EPA) is cleaning up the contamination at the Gelman Sciences, Inc. Superfund site in your community.  The U.S. EPA believes the active, meaningful involvement of community members is critical to the success of a cleanup effort.  This survey is an opportunity for you to tell us how well we are doing at listening to your concerns about the cleanup and making it possible for you to participate in the planning and decision making process.  Please take a few minutes to answer the questions.  Your views are important and will help us to be more responsive to your needs and interests.  

This survey is being conducted in accordance with the Federal Paperwork Reduction Act Information Collection Request # 1487.13.  You will need about 15 minutes to answer the questions.
