Notification Forms for Underground Storage Tanks
The 2015 UST regulation in 40 CFR 280.22 (Notification Requirement) requires owners to notify their designated state or local agency or department in two instances.
- Within 30 days of bringing an underground storage tank into use, owners must submit the Notification for Underground Storage Tanks form (EPA Form 7530-1; Rev 6-2015) to their implementing agency.
- Within 30 days of assuming ownership of a regulated UST, owners must submit the Notification of Ownership Change for Underground Storage Tanks form (EPA Form 6200-10) to their implementing agency.
The two forms, along with additional guidance, are below. Note that some states may have their own version of the notification forms. Owners should check with their implementing agency.
For UST owners in Indian country where EPA is the implementing agency, EPA contact information and mailing addresses are provided below.
