RAD Annual Reporting Form

Responsible Appliance Disposal (RAD) Program logo

RAD Partners must submit annual reports on available program information, including:

  • Number and age of appliances collected
  • Type and quantity of refrigerant recovered and subsequently reclaimed or destroyed
  • Type and quantity of foam blowing agent recovered and subsequently reclaimed or destroyed
  • Weight of metals, plastics, and glass recycled
  • Quantity of hazardous waste products and used oil recovered or destroyed
  • Energy savings information associated with retirement of old appliances (if applicable)
  • Instructions on downloading the Excel form.

Reporting forms should be completed electronically using the  2022 RAD Partner Reporting Form (xlsm) (updated 12/2022).

RAD Reporting Form Downloading Instructions

If you have problems opening the Excel version of the reporting form, please follow these instructions:

  1. Open Excel
  2. Select "Tools" (make sure you see the entire menu)
  3. From the drop-down menu, select "Macros"
  4. From the sub-menu, select "Security"
  5. Under the "Security Level" tab, de-select "High" and select "Low"
  6. Press "OK"
  7. Open the RAD Reporting Form. After opening the file and saving it to your computer, it is recommended to set the security level back to “High” following the same steps above.

Completed forms should be sent by email to Sally Hamlin at hamlin.sally@epa.gov by March 3, 2023.
