Certification Statements for Risk and Resilience Assessment or Emergency Response Plan

As an alternative to certifying electronically, community water systems may use the PDF certification statements below to certify completion of the Risk and Resilience Assessment and/or Emergency Response Plan. The statements can be sent to EPA via the following methods:

1. Email signed copy to U.S. EPA at awiaservicedesk@epa.gov

2. Mail signed copy to U.S. EPA at the following address:

U.S. EPA Data Process Center
ATTN: AWIA
C/O CGI Federal
12601 Fair Lakes Circle
Fairfax, VA 22033

Please ensure that your Risk and Resilience Assessment and Emergency Response Plan are compliant with the requirements set in America’s Water Infrastructure Act (AWIA) section 2013/Safe Drinking Water Act (SDWA) section 1433 prior to submitting the certification statements. Please do not submit your actual Risk and Resilience Assessment or Emergency Response Plan to EPA with your certification statements; please keep your Risk and Resilience Assessment or Emergency Response Plan documents in a secure location at your water system for at least five years.
