Federal regulations require generators and transporters of hazardous waste and owners or operators of hazardous waste treatment, storage, or disposal facilities (TSDFs) to use the uniform hazardous waste manifest (EPA Form 8700-22) and, if necessary, the continuation sheet (EPA Form 8700-22A) for both interstate and intrastate transportation.
EPA is providing below in PDF format a sample of what the uniform hazardous waste manifest and continuation sheet look like as well as the instructions for completing the uniform hazardous waste manifest.
Please note that the form, continuation sheet and instructions including the manifest distribution scheme are in the process of being updated to reflect the e-Manifest regulations and implementation of the e-Manifest system. Once they have been finalized, we will update this web page with examples of those new forms.
