UIC Permitting and registration information

UIC INVENTORY INFORMATION:

For locations and well classes where USEPA Region 2 directly implements the UIC program, the USEPA Region 2 UIC program has developed "USEPA Region 2 Supplemental Instructions for Completing EPA Form 7520-16 (Rev. 4-19)," and a more comprehensive list of Well Types based on USEPA reporting codes “USEPA Region 2 List of Injection Well Classes & Types.”

Please complete and submit inventory information via the “Owner or Operator Online Form for Inventory of Injection Wells (7520-16).” Owners and operators are to read and refer to the following resources when filling out EPA Form 7520-16.

IMPORTANT: Additional Information requirements are outlined in the Region 2 Supplemental Instructions. Submit the Additional Information via email to region2_uic@epa.gov. On the subject line, include Facility Name, Facility Address, the subject of the e-mail, and UICID Number if known. Should you know the current Case Handler for the Facility, also cc the Case Handler.

Failure to read and refer to the Region 2 Supplemental Instructions and List of Injection Well Classes & Types or to submit both EPA Form 7520-16 and the Additional Information could delay the inventory review process.


UIC Class V Closure Information

The purpose of the UIC program, promulgated under part C of the Safe Drinking Water Act, is to protect  Underground Sources of Drinking Water (USDWs.) Owners or Operators of Class V wells are responsible for complying with the EPA UIC Regulations in 40 C.F.R. Parts 144 through 147. As per §§144.12(a) and 144.82(a), prohibition of movement of fluid into USDWs, no owner or operator shall construct, operate, maintain, convert, plug, abandon, or conduct any other injection activity in a manner that allows the movement of fluid containing any contaminant into a USDW, if the presence of that contaminant may cause a violation of any primary drinking water regulation under 40 C.F.R. Part 142 or may otherwise adversely affect the health of persons (endangerment).

As per §144.82(b), closure requirements, owners or operators must close the well in a manner that complies with the above prohibition of fluid movement. Also, owners or operators must dispose of or otherwise manage any soil, gravel, sludge, liquids, or other materials removed from or adjacent to the well in accordance with all applicable Federal, State, and local regulations and requirements. Closures of large-capacity cesspools (LCC) and motor vehicle waste disposal wells (MVWDW) are required under additional regulations (see, 40 C.F.R. §§ 144.84, 144.85, 144.88 and 144.89). Both LCCs and MVWDWs are currently banned. MVWDWs and LCCs are defined at 40 C.F.R. 144.81(2) and 144.81(16), respectively.

IMPORTANT: For voluntary closures of Class V wells without individual permits that are being overseen by a qualified environmental professional use:

  • Be sure to submit the signed Certification with updated inventory at the completion of the closure work and when the wells are permanently closed.

For closures of Class V wells conducted under an enforcement action, please refer to guidance provided by the Enforcement and Compliance Division.
