8. What steps do I take after the proficiency testing?

After laboratories report their data to the Proficiency Test (PT) Providers, the PT Provider issues a report to the laboratory and the state DMR-QA coordinator, indicating the results were "Acceptable" or "Not Acceptable". The laboratory must then forward a copy of the graded report to the permittee. The permittee subsequently completes the checklists included in the DMR-QA Study package, indicating the laboratory's grade for each required analyte. One set of checklists must be used for each laboratory. The permittee must also fill out the NPDES Permittee Data Report Form (EPA Form 6400-01, included in the DMR-QA study package) and submit a signed copy with the completed checklists and copies of the laboratory's graded reports to the state coordinator by the due date specified on the form.
