7. What if I need to change or amend an initial or annual production report that I have already submitted?

For reports that have previously been submitted electronically and accepted by EPA, please use the link to access our How-to Guide to amend EPA Form 3540-16. 

For reports that have previously been submitted by mail, download, fill and resubmit by mail EPA Form 3540-16 Pesticide Report for Pesticide-Producing and Device-Producing Establishments. Clearly indicate on the form that the submission is an amended report and the reporting year being amended.
