Chromium Finishing Questionnaire
The EPA is administering the Chromium Finishing Questionnaire as part of its effort to review and revise, as appropriate, the effluent guidelines and standards for the Metal Finishing Category (40 CFR Part 433) and Electroplating Category (40 CFR Part 413). The questionnaire will collect information and data on facilities that conduct certain chromium-related metal finishing and electroplating activities that the EPA has determined may use and/or discharge PFAS. Chromium finishing operations that are the focus of this data collection effort include chromium plating, chromium anodizing, chromic acid etching, chromate conversion coating, and any other metal finishing or electroplating process that deposits a layer of chromium on any base metal/material or uses chromium materials in the fabrication of a metal product.
If you received a notification letter from the EPA, you are required by federal law to complete and submit the Chromium Finishing Questionnaire on behalf of your facility. Companies operating multiple applicable facilities are required to complete and submit a questionnaire for each individual facility. Facilities that have not engaged in chromium finishing operations at any time since 1995, have permanently closed by January 2023, or have permanently discontinued all metal finishing and electroplating by January 2023 are only required to complete a few questions.
Log In to Your Facility’s Qualtrics Questionnaire
To access the online questionnaire, click the “Take Me To My Questionnaire” button below which will redirect you to the Qualtrics login webpage. Then, enter the Questionnaire ID and entry code assigned to your facility in the notification letter received from the EPA. Additional information and instructions are available in Qualtrics following the login webpage.
If your company operates any facilities in the United States that currently or previously conducted these chromium finishing operations and these facilities did not receive a notification letter, contact the EPA Chromium Finishing Questionnaire Helpline (see contact information in the Technical Support section below) to request copies of the questionnaire for these facilities. If you believe your facility is ineligible for this questionnaire, you cannot access the questionnaire using the Questionnaire ID and entry code provided in the notification letter mailed to you from the EPA, or you received multiple notification letters for the same facility, contact the Chromium Finishing Questionnaire Helpline.

The Helpline is operated by Eastern Research Group, Inc. (ERG), an EPA contractor, from 9:00 AM to 4:00 PM Eastern Time from Monday to Friday. Both phone lines require you to leave a voicemail, and Helpline staff will respond to your inquiry as soon as possible. When contacting the Helpline, include your name, business phone number or email address, the name and Questionnaire ID of your facility, and a detailed description of your inquiry in your voicemail or email.
