3. How do I register my pesticide-producing and device-producing establishment?
Request a company number, if you have not previously obtained one. This process can only be completed electronically. Once you have obtained a company number you can submit your application to register an establishment electronically (recommended) or by mailing a completed EPA Form 3540-8.

For paper submissions, download, complete, print, and sign EPA Form 3540-8: Application for Registration of Pesticide-Producing and Device-Producing Establishment. Please read the instructions for EPA Form 3540-8 available at the link above for information on how to complete and mail the form.
EPA may contact you with questions regarding your application. If there are no questions and the application appears accurate and complete, you will receive an automated email at the address provided in your application when EPA completes its review. The email will confirm the registration of the establishment and provide you with the EPA establishment number.
It is unlawful to knowingly falsify all or part of any application, information, records and reports submitted to EPA under any provision of FIFRA. For additional assistance, contact the pesticide establishment coordinator where your company headquarters is located.
Note: You must report any changes in company, establishment, or agent information to the appropriate pesticide establishment coordinator in writing on company letterhead or on EPA Form 3540-8 within 30 days after such change occurs.
