Federal permits are required for all Class I, II, III, VI wells, and some Class V wells. For a permit application or related information, please see Region 9 contacts or visit the UIC Reporting Forms site. Region 9 states that have delegated programs have permit requirements that vary by jurisdiction and well type. Injection well owners are responsible for determining all applicable requirements. Permit requirements can be found in 40 CFR parts 124, 144 and 146.

EPA provides access to these materials consistent with the Agency's procedures for handling confidential business information (CBI) under 40 C.F.R. Part 2, Subpart B. Certain modeling files are not provided due to their file format. If noted, information provided by the applicant has been claimed as CBI and is not publicly releasable under FOIA.
