Annual Report Information

The 2003 Massachusetts and New Hampshire MS4 permit requires submission of annual reports. Reports must be submitted to both EPA Region 1 and the appropriate state agency on May 1st each year throughout the term of the Permit. The latest version of the suggested format can be downloaded here: Year 15 Annual Report Suggested Template (MS Word) (16 pp, 56 K). The purpose of the annual report is to document the status of Storm Water Management Program (SWMP) implementation. Annual Report requirements can be found in Part V.F. of the 2003 MA and NH MS4 Permit and must be signed and include a certification in accordance with Part VI.G. of the Permit.


Annual Reports may be submitted electronically to EPA OR by mail to EPA and the appropriate State.
